The Healthy Communities Database is maintained by the National Association of Counties (NACo) and is supported by the Robert Wood Johnson Foundation. The Healthy Communities Database is a model practice database that includes practices and initiatives which counties have implemented to prevent childhood obesity. Topics include: nutritious diets, physical activity, built environments, and social environments.
User Functionality
Users can search for model practices by state, county size (i.e., small, medium, large), issue areas, practice type (i.e., initiative, policy, program), or keyword. Information includes a summary of the model practice, county-specific information, as well as contact information for the model practice.
