By signing this form and obtaining the requested data sets from the Primary School Deworming Project (PSDP), the User of any PSDP public use data agrees: [1] To use the data sets solely for research purposes, including statistical reporting and analysis. The data sets cannot be used for commercial ends, nor can they be sold; [2] To maintain a secure computing environment for storage and use of these data sets and any data sets derived from them. Users agree not to share these data with, or provide copies of these data to, any other person or organization, and to return or destroy these data sets, and any derivative data files, upon request from Primary School Deworming Project (PSDP) administrators; [3] To obtain relevant IRB and possibly other required approvals before using PSDP data; [4] To make no attempt to link these data sets with individually identifiable records from any source, or in any other way attempt to identify the persons in any PSDP data sets; [5] To not publish results (in map or other form) that would allow communities or individuals to be identified; [6] If the identity of any person or establishment in these data sets is inadvertently discovered, then (a) no use will be made of this knowledge, (b) PSDP administrators will be notified immediately (Edward Miguel (emiguel@berkeley.edu) and Joan Hamory Hicks (jrhamory@berkeley.edu)), (c) the information that would identify any individual or establishment will be safeguarded or destroyed, as requested by the PSDP administrators, and (d) no one else will be informed of the discovered identity; [7] To cite the PSDP as the data source in any publications or research based upon these data and to acknowledge the funding source of the PSDP.
